	Step 1: PRINT Download and print the form and fill out questions that you can answer from your kitchen table. If you prefer, you can also fill out the form and submit online or call 800-695-7375 and request that a form be mailed to you.
	Step 2: ASSESS Here’s the fun part. Grab your tool belt and get ready to get cozy with your home. Be prepared to peek into your attic space and dive into your utility closet to answer questions on insulation and water heating systems.
	Step 3: MAIL After completing your energy audit, make sure you have your Questar Gas account and service-agreement numbers written on the form (you can find this information on your gas bill) and either e-mail the form to ThermWise.Audit@Questar.com or send to: ThermWise Home Energy Audit Program PO Box 45360 Salt Lake City, Utah 84145-0360
